Quantum also offers Voluntary Life Insurance as an optional benefit. Employees pay a monthly premium for this coverage. Although the employee pays for this coverage, because it is an employer sponsored benefit, the monthly premium is usually more affordable than what you can purchase with a standalone policy; Within the first 30 days of employment, an employee may purchase up to $200,000, and up to $25,000 of spouse voluntary life with no Evidence of Insurability. Approval of new enrollment of an increase in Voluntary Life coverage outside the new hire enrollment period is subject to Evidence of Insurability and medical underwriting performed by Guardian.

DISABILITY INSURANCE
Quantum offers voluntary short and long-term disability insurance to employees through Guardian. In the event that you become disabled from a non-work-related injury or sickness, disability income benefits are provided as a source of income.

Short Term Disability (VOLUNTARY)
Elimination Period: 8 days due to accident or illness
Benefit Period: 12 Weeks
Weekly Benefit: 60% of base earnings (up to $1,000 per week)

Long Term Disability (EMPLOYER PAID)
Elimination Period: 90 days
Benefit Period: Social Security Normal Retirement Age
Weekly Benefit: 50% of base earnings (up to $3,000 per month)

Buy-Up Long Term Disability (VOLUNTARY)
Elimination Period: 90 days
Benefit Period: Social Security Normal Retirement Age
Weekly Benefit: 60% of base earnings (up to $7,000 per month)
